Overview

Semax is a synthetic heptapeptide with the sequence Met-Glu-His-Phe-Pro-Gly-Pro (MEHFPGP). It is derived from the ACTH(4-7) fragment of adrenocorticotropic hormone - a four-amino-acid sequence at the N-terminal domain of ACTH - extended at the C-terminus with the tripeptide Pro-Gly-Pro to substantially increase metabolic stability and extend its duration of bioactivity compared to the unmodified ACTH fragment. Semax was first described in scientific literature in 1991 and was developed at the Institute of Molecular Genetics of the Russian Academy of Sciences. It has been registered as a pharmaceutical in Russia and Ukraine since 1994 for the clinical treatment of ischemic stroke, dyscirculatory encephalopathy, optic nerve atrophy, and cognitive disorders.

A critical structural distinction separates Semax from full-length ACTH: despite retaining the neurotrophic properties of the ACTH(4-10) fragment, Semax lacks the hormonal activity of the complete ACTH molecule and does not stimulate cortisol production from the adrenal glands. This makes it pharmacologically unique as an ACTH-derived compound without HPA axis involvement. Its primary studied mechanism involves upregulation of brain-derived neurotrophic factor (BDNF) and its receptor TrkB in the hippocampus, alongside modulation of serotonergic, dopaminergic, and cholinergic neurotransmitter systems. Semax has also been shown to inhibit enzymes responsible for enkephalin degradation, a mechanism shared with Selank.

Origin & Design: Derived from the ACTH(4-7) neurotrophic fragment of adrenocorticotropic hormone. The C-terminal Pro-Gly-Pro tripeptide was added to extend metabolic stability from minutes to approximately 20-24 hours, making it viable as a research and clinical compound. Does not produce cortisol release or other hormonal effects of full ACTH.

Research Context: Approved as a prescription drug in Russia and Ukraine for stroke and cognitive disorders. Human clinical data exists from Russian controlled trials, primarily in stroke recovery populations. The majority of published research originates from Russian institutions and has limited independent Western replication. Widely studied for BDNF upregulation, neuroprotection, and cognitive function in animal models.

Storage Requirements

Semax is supplied as a lyophilised (freeze-dried) solid. As a heptapeptide containing methionine, Semax is susceptible to oxidative degradation and requires careful handling to prevent methionine sulphoxidation, which can reduce biological activity. Follow the specifications below for optimal compound integrity.

Temperature: -20°C Long-term storage. For short-term use (up to 4 weeks), 2-8°C is acceptable.

Light Sensitivity: Light-Sensitive. Store in original opaque vial. Avoid direct UV or prolonged exposure to light.

Shelf Life: 24 Months. Lyophilized, sealed, stored at -20°C. Batch expiry printed on vial label.

Preparation Notes

  1. Allow the sealed vial to reach room temperature before opening to avoid condensation on the lyophilised cake.
  2. Use bacteriostatic water (BAC water) or sterile saline as the reconstitution solvent. Semax is water-soluble and dissolves readily in aqueous solution. Add solvent slowly down the inside wall of the vial - do not inject directly onto the lyophilised cake.
  3. Gently swirl until fully dissolved. Do not vortex or shake vigorously. Semax dissolves easily and does not require heat or prolonged agitation.
  4. Once reconstituted, store at 2-8°C and protect from light. Use within 14 days. Degradation of reconstituted Semax is accelerated compared to the lyophilised form. Do not freeze reconstituted solution.
  5. Discard if solution appears cloudy, discoloured, or contains particulate matter.
