Overview

Tirzepatide (LY3298176) is a synthetic 39-amino acid peptide that simultaneously activates both the glucose-dependent insulinotropic polypeptide receptor (GIPR) and the glucagon-like peptide-1 receptor (GLP-1R). Developed by Eli Lilly, it is structurally based on the native GIP sequence, with modifications enabling dual receptor engagement. A C20 fatty diacid moiety is attached via a linker to enable albumin binding, extending its half-life to approximately 5 days and allowing once-weekly subcutaneous dosing. Researchers describe it as a "twincretin" - a single unimolecular dual agonist that does the work of two incretin hormones.
