Overview

Sermorelin is a synthetic 29-amino acid peptide that corresponds to the N-terminal 1-29 fragment of endogenous human growth hormone-releasing hormone (GHRH). The native human GHRH molecule is 44 amino acids long, but in vitro work demonstrated that the first 29 residues retain full biological activity at the GHRH receptor, making sermorelin (also designated GHRH(1-29)NH₂) the shortest fully functional fragment of the parent hormone. It was originally developed by Serono and approved by the FDA in 1997 under the brand name Geref for the diagnosis and treatment of growth hormone deficiency in children. Geref was withdrawn from the US market by the manufacturer in 2008 due to a supply issue with the active ingredient, not for safety or efficacy reasons.

Mechanistically, sermorelin binds the GHRH receptor on somatotroph cells of the anterior pituitary, activates Gαs, and elevates intracellular cAMP, which triggers pulsatile release of endogenous growth hormone (GH). Critically, this differs from exogenous recombinant human GH (rhGH) administration in that sermorelin works through the body's existing feedback architecture: GH and downstream IGF-1 still feed back via somatostatin to limit excessive secretion, and the hypothalamic-pituitary axis remains intact. Sermorelin's terminal half-life is approximately 11-12 minutes, requiring frequent dosing in clinical use. It is now most commonly investigated as a research tool in adult growth hormone insufficiency, age-related GH decline, body composition modelling, and sleep architecture studies.

Mechanism: Selective GHRH receptor agonist on pituitary somatotrophs. Activates adenylate cyclase / cAMP / PKA signalling, triggering physiological pulsatile GH secretion. Downstream, hepatic IGF-1 production rises in response to elevated GH exposure. Negative feedback via somatostatin remains intact.

Research Context: FDA-approved 1997 for pediatric GH deficiency (Geref); withdrawn 2008 due to manufacturing supply. Off-label investigated in adult GH deficiency, age-related somatopause, and HIV-associated lipodystrophy. Compounded sermorelin remains accessible to licensed prescribers in some jurisdictions; closely related GHRH analogue tesamorelin retains approval for HIV lipodystrophy.

Storage Requirements

Sermorelin is supplied as a lyophilised (freeze-dried) sterile, non-pyrogenic powder as the acetate salt. The peptide is susceptible to enzymatic cleavage at the Arg11-Lys12 and Lys12-Val13 bonds in solution and requires careful handling to preserve integrity. Follow the specifications below.

Temperature: -20°C Long-term storage. For short-term use (up to 4 weeks), 2-8°C is acceptable.

Light Sensitivity: Light-Sensitive. Store in original opaque vial. Avoid direct UV or prolonged exposure to light.

Preparation Notes

  1. Allow the sealed vial to reach room temperature before opening to minimise condensation and moisture introduction.
  2. Use bacteriostatic water (BAC water) or sterile sodium chloride 0.9% as the reconstitution solvent. Add solvent slowly down the inside wall of the vial - do not inject directly onto the lyophilised cake.
  3. Gently swirl until fully dissolved. Do not vortex or shake vigorously, as agitation can disrupt the peptide structure.
  4. Inspect the solution: it should be clear and colourless with no visible particulates. Discard if cloudy, discoloured, or contains particulate matter.
  5. Once reconstituted, store at 2-8°C protected from light. Use within 14-28 days. Do not freeze reconstituted solution
